We can guide and support you to self-manage your diabetes.
At DPV Health the Diabetes Nurse Educator will work with you, your family/carers and the diabetes team to assist you to:
- Understand what is diabetes
- Learn how diabetes affects your body
- Learn how to test blood glucose levels
- Learn what affects your blood glucose level, and strategies to achieve your target levels
- Understand hypoglycaemia; prevention and management
- Manage your blood glucose, when unwell
- Learn about how your medication and insulin work
- Teach yourself how to take your medication and insulin
- Support you to start injections
- Understand the importance of a healthy lifestyle and learn to live well with diabetes
- Register with NDSS to access subsidised supplies
- Referral to other members of the diabetes team
The Diabetes Nurse Educator is experienced in the care of Type 1 Diabetes, Type 2 Diabetes and working with people at risk of developing diabetes.
Fees
Please note there is a client co-payment for this service (exc NDIS & HCPs):
Healthcare Card / Pension Card holders: $10.30
Non Healthcare Card / Pension Card holders: Based on income range assessment.
Who can use this service
The service is available to all ages. GP/Hospital referral required.
Self-Managed Diabetes services are also available through NDIS and Home Care Package Plans
