Home Care Packages
DPV Health is an approved provider of Home Care Packages within Hume and Whittlesea. We offer the highest quality Home Care Package services, delivered by our experienced, caring, quality staff.

Aged Care Assist
Aged Care Assist provides intensive face-to-face and in-home support to help older people navigate My Aged Care, Home Care Packages and other services.

Falls Prevention
People often dismiss falls as ‘part of getting older’ or ‘just not concentrating’ but they are often a warning sign that something is not right, so it is important to discuss any fall with your doctor.

Social Support Programs
Our Social Support Programs assist people and their carers to maintain and enhance their wellbeing, prevent social isolation and maintain independence within their community.

Community Phone Connect Program
DPV Health can provide you with regular phone calls to check on your wellbeing. This provides peace of mind if you are at risk of an accident or illness that may go unnoticed. It's also a great way to feel connected and socialize.

Hospital Admission Risk Program
The Hospital Admission Risk Program (HARP) service at DPV Health manages people with chronic diseases or conditions, the elderly and seniors, and people with complex needs who frequently use hospitals or are at risk of hospitalisation.

Common health conditions associated with ageing
Common conditions in older age include hearing loss, cataracts and refractive errors, back and neck pain and osteoarthritis, chronic obstructive pulmonary disease, diabetes, depression, and dementia. Furthermore, as people age, they are more likely to experience several conditions at the same time.

The Bone Bus
Know how healthy your bones are.
Get your bone density checked with a mobile bone densitometry service providing accurate, reliable, and precise bone mineral density measurement for people aged 70 and over. The mobile Bone Bus will be available at both DPV Health Medical Centres:
Broadmeadows - 12th April until 15th April 2021 and Mill Park - 14th April until 15th April 2021.

Health Assessment
An annual health assessment will identify ways that you can improve your health and well being. It will help you by advising you on how you can maintain your independence at home and in your community.

Physiotherapy
Physiotherapists help to keep you well, strong and active. They can help to keep you independent and to manage pain through exercise and positive self-management.

Occupational Therapy
Occupational Therapists can help to promote your wellness, safety and independence. They work with people of all ages, and with a wide variety of conditions that affect the ability to remain independent and safe at home.

Counselling, Psychology & Social Work
At DPV Health, our staff of professional social workers, psychologists and mental health nurses are available to provide high quality counselling, psychology and social work services to you.

Dental Services
The DPV Health Dental Clinic offers a general dental practice and emergency service program within public sector guidelines.
