Ageing can be challenging, as you may experience new health conditions or need support in daily living. Our GPs can partner with you to take a proactive approach to your health. This helps you to build the life you want, maintain your independence, and detect any problems early.

Care Plans

A challenge of getting older is that you may have one or more chronic medical conditions. If so, you can benefit from a customised plan to coordinate your care.

A care plan is prepared by a GP, and provides a structured approach to care to achieve goals set by you and your doctor.

A chronic medical condition is a condition that lasts for more than six months, such as heart disease, diabetes, arthritis, stroke, cancer, and many other conditions.

To find out whether you are eligible for a care plan, book an appointment with your GP. Please book a long consultation so that you and the doctor have time to discuss your needs.

The care plan should be reviewed with your GP every six months to ensure that the plan is achieving your goals. GP appointments for creating and revising care plans are bulk-billed; however, some services recommended as part of the plan may incur a fee.

Depending on the type of care plan you need, you may be eligible for Medicare rebates for certain allied health services related to the management of your chronic condition. Ask your doctor for more information.

Health Assessments

We also offer annual comprehensive bulk-billed health assessments for people aged 75 and over who live in the community or in hostel-style aged care.

Fees

Bulk billing is available. All fees are set by individual doctors. Please enquiry about the fees at the time of making an appointment.

Medical services for refugees and asylum seekers are at no cost to the patient.
