
Diabetes Connect Program
Diabetes Connect program running at DPV Health until June 2025, where Type 2 Diabetes patients will have access to a multidisciplinary Diabetes Service in the community. We work with you, and your family/ carers to guide and support self-management of your diabetes.
What is the service?
- A community-based integrated care pathway for People with Type 2 Diabetes
- Aims to improve health and wellbeing outcomes for people with diabetes
- Reduce avoidable disease progression and hospitalisation
- The program aims to improve integration and coordination between Primary, Community and Acute Care Services
What sets us apart?
Within the program you will have the opportunity to have:
- A care coordinator supporting you throughout the program, be your initial point of contact and guide through services and systems
Access to Freestyle Libre sensor for at the beginning and end of the program if appropriate
Access to free Diabetes Nurse Educators, Allied Health, social support groups, and a range of additional programs and groups on offer at DPV Health
Note: all participants must consent to the completion of three 5-minute surveys throughout the program and sharing
of deidentified information for the evaluation of the pilot program.
Under the Diabetes Connect Program, you will be linked in with a care coordinator who will work with you, your family/carers to assist you with:
- Identifying current barriers to managing your health goals and linking in with allied health services, social services and, local groups and resources across Hume and Whittlesea communities
- Navigating the healthcare system and providing regular check ins and support throughout your time in the program
- Empowering you to take control of your health, improving your confidence in taking a lead role in your health goals and management
- Preventing avoidable risk of complications and progressions related to diabetes

- Diabetes Care Coordinator
- Diabetes Nurse Educators
- Dietetics
- Mental Health team/ Counselling
- Podiatry
- Exercise Physiology
- Physiotherapy
- Social Support groups and services
Endocrinology is available at DPV Health (this can be accessed under bulk-billing with a separate referral).
Fees
Diabetes Connect Program is a FREE service.
If you are referred to external services, fees may apply.
Who can use this service
This service is available to members of the community:
- Those with Type 2 Diabetes/ pre-diabetes
- Over the age of 18 years
You are not eligible for this program if you have type 1 diabetes or if you are already a part of the Hospital Admission Risk Program (HARP).
Access to Interpreter Service
We offer FREE interpreter access to all clients who need it.
If you require an interpreter, please let our staff know at the time of making an appointment.

We accept referrals from GPs, specialists or other health professionals.
