Diabetes
If you are at risk of diabetes, recently diagnosed, or have well-managed diabetes but would like support and education, we can help. We also work with other diabetes service teams.
Cardiac Rehabilitation
We have Cardiac Rehabilitation group programs available to you, which cover a range of topics including heart health, exercise, medications and healthy eating.
Women’s Health
Our Women’s Health Nurse provides screening and education, which includes a Cervical Screening Test, information relating to menopause, breast health, menstrual health, basic continence, contraception, fertility awareness, sexual health and other health issues that may be of concern to you. We also provide community education.
Our Community Health Nurse can help to improve your health and wellbeing, and provide referrals to other health and community services, if needed.
We are also trained to assist you if you have heart disease or diabetes; including if you are diagnosed with the disease or at risk of developing diabetes.
Fees
Healthcare Card / Pension Card holders: $10.10
Non Healthcare Card / Pension Card holders: Based on income range assessment.
Who can use this service
Our service is available to people of all ages. We accept self-referral or GP/Hospital or My Aged Care referral.
