How to Apply for a HCP

Sign Up in 6 Easy Steps!

All applications for government-funded aged care services are made through My Aged Care. Your DPV Health Care Manager will support you with information and guidance during this process.

Step 1. Apply for a HCP

Apply using My Aged Care online assessment (around 15-20 minutes to complete)

When you apply for your Home Care Package, advise the ACAS representative that you want DPV Health to be your care provider.

Step 2. Have a face-to-face assessment

You will have an assessment by the Aged Care Assessment Team (ACAT). You’ll discuss or show:

- What you can do well
- What you need some help doing
- What you want to be able to do

Step 3. Approved for a HCP

If approved, you will receive a My Aged Care Home Care Package Approval letter.

Your assessor will work with you to develop a personalised support plan to help identify what sort of services suit your needs.

Step 4. Wait to be allocated a package

You are placed on a national waiting list for allocation of a Home Care Package.

If you need services while you wait, contact us for help. There are many different ways to fund services while you wait for your Home Care Package.

Step 5. Review your agreement

Once you’ve been allocated your package, you’ll have 56 days to enter into a formal agreement with DPV Health. This document outlines things like:

- Costs and fees
- How your services will be provided
- Tailored Care Plan
- Client Handbook

Step 6. Monthly statements and ongoing service review

For your convenience, we will send you a financial statement of activity for the previous month.

You will also receive ongoing support and advice from your local DPV Health Home Care Team. Your Care manager is your key contact for anything related to your care. If your needs change, we will review your services and talk to you about your options.

Fees

Each level of home care package provides for a different subsidy which is paid to your selected approved home care provider. The subsidy and rates will change periodically, and you will be notified when this occurs.   

If you have a Home Care Package, you may also be eligible for additional supplements for some specific care needs or if you are having difficulty paying your aged care fees.

To receive a Home Care Package, you’ll need an assessment to see if you’re eligible.

If you haven’t been assessed for a Home Care Package yet, you (or your nominated representative) need to call My Aged Care on 1800 200 422.

The Australian Government pays for most of aged care costs in Australia. If you are a full pensioner, you do not need to contribute towards a Home Care Package. If you are a self-funded retiree, Centrelink will advise you on the amount you are required to contribute.

Yes, we have a Care Finder team member who can support people who are eligible for aged care services and require intensive support to either interact with My Aged Care or access other community-based supports. To contact a Care Finder Please call 0438 849 400.

Yes. DPV Health can assist you with an interpreter. Please let us know that you’d like an interpreter.

Level 1 Home Care Packages are designed to support people who have basic level care needs. This may include help with everyday tasks such as personal care, domestic assistance, social support, meal preparation and transport.

Level 2 Home Care Packages are designed to support people who have low level care needs. This may include help with everyday tasks such as personal care, domestic assistance, social support, meal preparation, transport and some nursing care.

Level 3 Home Care Packages are designed to support people who have intermediate level care needs. This may include help with nursing care, medication management, allied health support, personal care, domestic assistance, social support, meal preparation, transport and support with changes to memory and/or behaviour.

Level 4 Home Care Packages are designed to support people who have high level or complex care needs. This may include help with nursing care, medication management, allied health support, personal care, domestic assistance, social support, meal preparation, transport and support with changes to memory and/or behaviour.

The Australian Government pays for the bulk of aged care in Australia. However, to receive help at home through the government’s home care packages you may need to contribute towards a means-tested fee, payable if your income is over a certain amount.

Once you’ve had an ACAT assessment, this will be sent to Centrelink, which will determine how much you will need to pay.

The Commonwealth Home Support Program (CHSP) provides entry-level support for older people who are still able to manage living at home, but could do with some extra assistance. Services are prescribed by a Regional Assessment Service (RAS).

Home Care Packages (HCP) provide higher levels of support for older people who have more complex needs. Recipients are able to select what services they want to use their funding on.

DPV Health works with a number of carefully selected and monitored local providers that provide the direct care workers whom visit the clients in their own homes. This allows DPV Health a broader access to direct care workers to match with clients’ needs (e.g. language skills, gender, medication or dementia training). All DPV Health and contracted service provider staff have appropriate training, qualifications and police checks.

DPV Health requires 24 hours’ notice for cancellations of services. Please let the Service Coordinator know of any cancellations or changes to your care needs as soon as possible.
