Community Phone Connect Program

Do you want to feel more connected?

Ensuring that you stay healthy & safe at all times.

The Community Phone Connect Program is a joint initiative between DPV Health, Broadmeadows Police, Hume City Council and Whittlesea City Council has been in operation since 2006.

This is a free community service for older people and people with disability, living on their own in the Hume and Whittlesea region.

Through the Community Connect Program, you will receive regular phone calls to check that you are safe, secure and well.

How does it work?

A volunteer will call you, to check on your wellbeing, on the day of the week that you have requested and ask if you have any safety concerns.

There will also be an opportunity for a friendly chat, if you like.

If the volunteer is unable to make contact with you on your scheduled day for a wellbeing phone call, the person/s you nominated and listed on your registration form, will be contacted. If your nominated person/s is also not contactable then Victoria Police will be called for a welfare check to be made at your home.

How do I join?

If you are a resident of Hume or Whittlesea and over the age of 50, living on your own and/or have a disability, just complete the application form on the brochure (click here to download) and send it to:
