Hospital Admission Risk Program

Do you need support with monitoring your health?

We can help clients who may have unstable chronic respiratory disease, chronic heart failure, diabetes, or other chronic diseases/conditions.

The Hospital Admission Risk Program (HARP) service at DPV Health manages people with chronic diseases or conditions, the elderly and seniors, and people with complex needs who frequently use hospitals or are at risk of hospitalisation.

Our HARP team works together to improve outcomes for our clients, provide integrated care across hospitals and the community, reduce avoidable hospital admissions and emergency department presentations, and improve access to healthcare.

Assistance is also given to people who may have complex psychological needs related to depression and anxiety, drug and/or alcohol problems, hormelessness or at risk of homelessness, and people with mental health issues.

Our clients with complex needs may include people with self-management issues, those who have a fragile support system or are experiencing carer stress, clients with medication management issues, and people who live alone and are not coping.

The DPV Health HARP team consists of the following health professionals:

  • Endocrinologist
  • Dietitian
  • Physiotherapist
  • Podiatrist
  • Counselling services
  • Community nurses who specialise in diabetes; Diabetes Nurse Educator, Mental Health, Respiration and Cardiac Care

Frequently Asked Questions

Eligible clients for HARP services include people who are:

At imminent risk of hospitalisation
Have had an avoidable hospital admission in the past twelve (12) months and requires moderate-to-intensive care coordination

HARP services are fully government-funded and free to all eligible clients.

Our team will review your situation and advise if you are eligible for HARP services, as you must meet certain requirements.

We accept self-referrals, also referrals from your GP, hospital or other health professionals, carer or family member.

For current clients who would like to book or cancel appointments, contact Reception on 9409 8787.

Please bring the following to your appointment:

Name and contact details of your regular GP
Reports of relevant scans or blood tests
A list of medications you currently use

Remember to allow up to one (1) hour for your first appointment.

If you consent, you may be referred to other HARP services following your first appointment, as further health needs may be subsequently identified.
