Are you at high risk of hospitalisation due to a chronic illness? Our Hospital Admission Risk Program (HARP) improves patient outcomes and reduces healthcare costs. We develop targeted care plans and interventions to prevent hospitalisation or readmission, which can help to improve patient outcomes and reduce healthcare costs.

The HARP service at DPV Health manages people with chronic diseases or conditions, the elderly and seniors, and people with complex needs who frequently use hospitals or are at risk of hospitalisation.

Our HARP team works together to improve outcomes for our clients, provide integrated care across hospitals and the community, reduce avoidable hospital admissions and emergency department presentations, and improve access to healthcare.
