Pathway to Good Health

Supporting the health and wellbeing of vulnerable children.

To ensure that vulnerable children entering or re-entering the Out-of-Home-Care system (OoHC) for the first time and all young people residing in residential care receive the best possible health care. A model of targeted care has been developed and funded by the Victorian Department of Health, in collaboration with DFFH. For more information click here.

OoHC is a temporary, medium or long-term living arrangement for children and young people who cannot live in their family home.

Our Pathway to Good Health clinic at DPV Health is based at our Broadmeadows and Mill Park sites, our specialist multidisciplinary team includes a Paediatrician, Nurse Coordinator, Speech Pathologist and Child Psychologist.

The role of the Pathways to Good Health service is to provide a comprehensive health and developmental assessment of the child/ young person and develop a health management plan that  includes recommendations to meet health needs. We aim to provide coordinated, collaborative care that is trauma sensitive.

Children/ young people are referred to the Pathway to Good Health service by the Child Protection Case Manager working with the child.

Who can use this service?

Children are referred to the Pathway to Good Health clinic by the Child Protection Case Manager, our Nurse Coordinators will reach out to caregivers on entry to care.

How much does it cost?

This is a free service.
