Pathways to Good Health

Supporting the health and wellbeing of vulnerable children.

To ensure that highly vulnerable children entering the Out-of-Home-Care system (OoHC) for the first time and all young people residing in residential care receive the best possible health care to address health deficits, a model of coordinated care and health planning was developed and funded by the Department of Health & Human Services (Victoria) known as the Pathways to Good Health. OoHC is a temporary, medium or long-term living arrangement for children and young people who cannot live in their family home.

Our Pathways to Good Health clinic at DPV Health is based in Broadmeadows and contains a specialised team that includes a Paediatrician, Nurse, Speech Pathologist and Child Psychologist.

The role of the Pathways to Good Health team is to provide a comprehensive assessment of the child and develop a health management plan that will include any diagnosis and recommended treatment or referrals.

Children are referred to the Pathways to Good Health clinic by the child protection or Out of Home Care agency working with the child.
