Do you have an ongoing health issue that you find difficult to manage, such as diabetes, arthritis, obesity, heart disease or chronic pain? Are there things you would like to do to improve your health, but you find it hard to get motivated? Not sure where to start? Then a Health Psychologist can help you. We can work with you to help improve your health and wellbeing, provide support and motivation to make positive changes, and connect you to other health and wellbeing services.
Our Health Psychology services are available to adults aged 18 years and over, living in the City of Whittlesea, who have one or more chronic health conditions. This could include (but is not limited to) diabetes, heart disease, lung disease, obesity and chronic pain.
Our Health Psychologist can:
- Provide one-on-one psychological assessment and strategies for psychological wellbeing
- Develop education and behavioural change programs
- Help you to identify your barriers to achieving better health, and how to overcome barriers such as anxiety, depression, anger
- Provide information about relevant services that may assist you and/or refer you to other relevant services, such as a Physiotherapist or Dietitian
We will help you to work on what matters to you. We might give you guidance or make suggestions, but it is up to you what actions you take.
Fees
Healthcare Card / Pension Card holders: $10.10
Non Healthcare Card / Pension Card holders: Based on income range assessment.
Who can use this service
The service is available to all ages. GP/Hospital referral required.
