eReferral Project

DPV Health is the lead agency for the second phase of a Department of Health and Human Services (DHHS) funded project that will improve referral processes between health services and General Practitioners (GPs) in the Northern region.

Electronic referrals (eReferrals) are recognised as the foundation to improving care across healthcare settings by providing a platform for efficient transmission of referral information. GPs and Practices from the Cities of Hume and Whittlesea currently using compatible versions of Best Practice and Medical Director are able to send secure eReferrals to DPV Health and Northern Health using the eReferral SmartForms solution by HealthLink.

HealthLink SmartForms:

  • require few steps to generate and send an eReferral to DPV Health and Northern Health services
  • automatically writes a copy of the sent referral back into the patient record in the GP’s Clinical Software
  • acknowledge sent eReferrals.

Where is the project up to?

  • GPs in the Cities of Hume and Whittlesea have successfully sent over 450 eReferrals to DPV Health and Northern Health.

What’s next?

  • Collecting Implementation Surveys from GPs to measure the improvements eReferrals have made to workflows within the Practice.
  • The Eastern Melbourne Primary Health Network and the North Western Melbourne Primary Health Network will continue to support Practices.

DPV Health to launch 30 March – New identity, same organisations

On 30 March 2018, Plenty Valley Community Health and Dianella Community Health are coming together to form one new combined organisation named DPV Health. The merger is a proactive move to improve services in the region. It will see the combined organisation achieve its growth potential and continue to service the growing needs of the community.

DPV Health will continue to provide clients with a positive healthcare experience. On 30 March, clients can expect to see the same highly professional staff, for the same quality services, at the same locations. This merger will benefit the community, and in the future, clients can expect to access services in more locations.

The DPV Health vision is ensuring health and wellbeing of our community.

Over a period of time you will see the DPV Health brand come to life, with the re-branding of locations and vehicles. The new and improved easy to use website will go live on 30 March 2018 (www.dpvhealth.org.au) along with its social media channels @DPVhealth (Facebook, LinkedIn, Twitter, YouTube and Google+).

In the meantime, you can visit the purpose built transition website (www.dchpvch.com) where you can post questions about the merger and register your email to be notified of updates.

DPV Health will provide a wide range of services for all ages including but not limited to medical, NDIS and disability, health and wellbeing, dental and aged care.
